TERMS OF PCA EMPLOYMENT ADDENDUM 1.A
Due to recent changes with the Minnesota Legislation, Department of Health (DOH) and Department of Human Services (DHS), there is new information we need to give to you and information we must get from you in order for you to remain an employee of any of Accurate Home Care Holdings, LLC’s (AHCH) companies, which include Accurate Home Care, LLC, Accurate PCA Services, LLC and Accurate Total Care, LLC.
The new law states that a PCA employee can not work more than 275 hours in any given month.
These hours are total hours in combination with ANY company you work for as a PCA (Personal Care Assistant).
For hours worked after July 1, 2009, you will not be paid if this form is not submitted to AHC by July 5, 2009
by fax, mail, email or drop off. You may also complete this form by accessing the home page of AHC’s website.
Completion of this form on the web will fulfill the requirement of returning this form.
